Student Nurse(SN)/Graduate Nurse (GN) Training Requirement Waiver 1101 Application

You are eligible to bypass the Wisconsin CNA training requirement if you meet the following criteria: 
  • You are currently enrolled in a Wisconsin Board of Nursing-approved pre-licensure program of nursing education, or you are enrolled in an approved program of nursing education in another state. DSPS’s lists of approved in-state nursing schools or approved out-of-state nursing schools. 
  • You have successfully completed a fundamentals of nursing course (or equivalent) and can provide a transcript. 
  • You will be required to successfully complete the Wisconsin competency examination.

  • I have successfully completed a fundamentals of nursing course (or equivalent) and 
    • I have uploaded a transcript from my school indicating my active status and successful completion of courses that teach basic nursing skills, including infection control, safety, emergency procedures, and personal care.
